The REACH Office has provided the following forms for student use. You may download/print the forms and submit them to the REACH Office. All forms are also available in the REACH Office. If you have any questions about form usage or problems accessing forms, please contact REACH at 877.237.0806.

Academic Information
| Intent to Graduate | Initiates the processes related to graduation and serves to inform Trinity of your plan for completion of all degree program requirements. Must be filed one calendar year prior to the date (semester) that you intend to complete all degree requirements. |
| Guidelines for filing the Intent to Graduate form | Instructions for accurate filing of the Intent to Graduate form. |
| Degree Completion Plan | Indicates schedule for completing remaining requirements. Must be filed along with the Intent to Graduate form. |
| Six-Hour Contract | Based on college catalog policy, request for permission to participate in the ceremony if you are within six hours of meeting graduation requirements. |
| Requests a letter of verification regarding student academic information. (Records is able to verify the following type of information for you: Graduation, Current Cumulative GPA, Full-time/Part-time status; Program Enrollment; Grades for Tuition Reimbursement, etc.) | |
| Pre-Approval of Transfer Credit Form - REACH (PDF) (Word) | Requests the evaluation of course work to be completed at a different school for credit toward your current Trinity degree program. For Organizational Leadership and Christian Ministry students. |
| Pre-Approval of Transfer Credit Form - REACH to Teach | Requests the evaluation of course work to be completed at a different school for credit toward your current Trinity degree program. For Elementary Education and Certificate students. |
Registration/TIU Access
| TIU Access | Register, add or drop your REACH courses online. |
Student Information
| Notifies REACH of a change to a student's address and/or information. The REACH office will be better able to serve you if you submit current contact information. | |
| Readmissions Checklist and Application | Guide for the readmissions process. For students who were enrolled previously but stepped out of the program for one year or more. |
Transcripts
| Transcript Request | Requests Trinity College transcripts. (NOTE: A hard copy or faxed signature is required for the release of a transcript.) |
